Prednisolone: Eye Drops, Orapred Liquid

Pred Forte is 1% acetate drops

PRED FORTE is prednisolone acetate ophthalmic suspension, USP, 1%. Each millilitre holds 10 mg of the acetate. The carton is a sterile topical anti-inflammatory for steroid-responsive inflammation of the palpebral and bulbar conjunctiva, cornea, and anterior segment. Shake well. Instill one to two drops into the conjunctival sac two to four times daily. During the first 24 to 48 hours the frequency may rise. If signs have not improved after two days, the sheet wants a re-evaluation. Do not stop the drops early just because the eye looks quieter.

AbbVie supplies opaque white LDPE bottles with pink HIPS caps: 1 mL in a 5 mL bottle, 5 mL in 10 mL, 10 mL in 15 mL, and 15 mL in 15 mL. Store upright, up to 25°C (77°F), and protect from freezing. The Irish HPRA leaflet adds that viral, fungal, and bacterial causes should already be ruled out before a short Pred Forte course.

Ten days of drops means IOP checks

Omnipred and generic prednisolone acetate sheets say prolonged topical steroid use can raise intraocular pressure, damage the optic nerve, cut visual fields, and form a posterior subcapsular cataract. If the product is used for 10 days or longer, intraocular pressure should be monitored even when that is hard in a child. Use caution if glaucoma is already present; check pressure often. Thin cornea or sclera can perforate under a topical steroid. Acute purulent infection can be masked. Viral eye disease, including herpes, can run longer and worse on a steroid drop.

DailyMed lists the common drop reactions as a rise in IOP with possible glaucoma, infrequent optic-nerve damage, posterior cataract, and delayed wound healing. Other reports: transient burning or stinging, foreign-body sensation, blur, eye pain, headache, pruritus, and rash. Mayo Clinic flags the sulfite in some prednisolone drops as an asthma trigger.

The cataract pack is three bottles, not Pred Forte

DailyMed's Strenza Plus kit is moxifloxacin 0.5% (one 3 mL bottle), bromfenac 0.09% (one 1.7 mL bottle), and prednisolone acetate 1% (one 5 mL bottle). It is a co-pack, not a single FDA-approved triple drop. Compounded prednisolone–moxifloxacin–bromfenac bottles exist after cataract surgery; Columbia Eye Clinic's printed card for that mix is three times daily for one week, then twice daily for one week, then once daily for one week, then stop. A 2020 OPTH contralateral-eye study used a different compounded mix, prednisolone acetate 1% plus gatifloxacin 0.5% plus bromfenac 0.075%, against the same drugs as separate drops. Pred Forte remains the 1% acetate suspension alone.

Orapred holds 15 mg in 5 mL

Orapred Solution is dye-free, pale to light yellow. Each 5 mL teaspoonful holds 20.2 mg prednisolone sodium phosphate, equal to 15 mg prednisolone base. DailyMed writes the same 15 mg per 5 mL line on generic sodium phosphate oral solution. The labelled adult start on that liquid runs from 1.67 mL to 20 mL a day, which is 5 to 60 mg prednisolone base, then holds or steps until a response shows.

Orapred ODT is a separate grape orally disintegrating tablet. The March 2024 FDA sheet lists 10 mg (as 13.4 mg sodium phosphate), 15 mg (as 20.2 mg), and 30 mg (as 40.3 mg). The 10 mg tablet is white, flat, beveled, debossed ORA on one side and 10 on the other; the 15 mg tablet carries 15. Cartons hold 12 tablets on two cards of six. Protect from moisture. Hypersensitivity to prednisolone or any tablet component is a contraindication.

Prelone is cherry 15 mg in 5 mL

Lannett's Prednisolone Syrup, USP, the Prelone reference, is a cherry-flavored red liquid with 15 mg prednisolone in each 5 mL. It is not the sodium phosphate salt. The bottle lists alcohol 5% v/v, cherry flavor, FD&C Blue No. 1, FD&C Red No. 40, and sucrose. Bottles are 240 mL (NDC 0527-5406-68) and 480 mL (NDC 0527-5406-70). The sheet's dose/volume chart on that 15 mg/5 mL strength is 15 mg = 1 teaspoon, 10 mg = 2/3 teaspoon, 7.5 mg = 1/2 teaspoon, 5 mg = 1/3 teaspoon, and it wants a calibrated measuring device, not a kitchen spoon.

Pediapred is a different liquid. The March 2024 FDA sheet is dye-free, colorless to light straw, raspberry. Each 5 mL holds 6.7 mg prednisolone sodium phosphate, equal to 5 mg prednisolone base, not 15 mg. The labelled start is 5 mL to 60 mL a day (5 to 60 mg base). Store 4° to 25°C; the bottle may be refrigerated. Aurobindo's 5 mg tablet is the Millipred reference, TE code AB, NDC 59651-491.

The liver converts prednisone to this steroid

LITFL's steroid chart lists prednisone and prednisolone at the same 5 mg equivalent, anti-inflammatory potency 4, mineralocorticoid 0.3, intermediate duration. Prednisone is the prodrug. The liver converts it to prednisolone, the active glucocorticoid. Madsbad's 1980 Gut study in 14 people with cirrhosis found mean serum prednisolone after oral prednisone was only 53% in the seven with severely impaired liver function versus the seven with milder impairment. Mean serum prednisolone after oral prednisolone did not move with liver function.

A 1979 Clinical Pharmacokinetics review called prednisolone the pharmacologically active species and noted some clinicians prefer it in active liver disease because conversion can be poor. The same review said delayed clearance can offset a lower converted level, so the advantage is not automatic. Conversion tables still print 1:1 milligram for milligram in people without that liver limit.

The peach tablet is scored P and L

Aurobindo's Prednisolone Tablets, USP 5 mg are peach, round, about 6.35 mm, flat beveled, scored, debossed P & L split by the score on one side and 5 on the other. Bottles of 50 (NDC 59651-491-50) and 100 (NDC 59651-491-01). Drugs.com's imprint card matches that peach 6 mm P L 5 tablet. Another USP 5 mg line is a scored yellow round debossed XJ / 011. Store 20° to 25°C. The carton is Rx only. DailyMed's adult start on the tablet is 5 mg to 60 mg a day by the disease, then hold or change until a response, or stop and switch if none comes.

What oral prednisolone treats

Mayo Clinic lists oral prednisolone for inflammation, severe allergies, adrenal problems, arthritis, asthma, blood or marrow problems, some eye disease, lupus, skin disease, kidney disease, ulcerative colitis, and multiple-sclerosis flares. It is a corticosteroid that works on the immune system to cut swelling, redness, itching, and allergic reactions.

DailyMed's tablet sheet opens the same prednisolone list at endocrine replacement (hydrocortisone or cortisone first in adrenal insufficiency), then short-term rheumatic adjuncts including rheumatoid arthritis, acute gouty arthritis, and bursitis, then collagen disease such as SLE, then severe allergic states that failed conventional treatment, including bronchial asthma and serum sickness. Ophthalmic disease on the oral sheet is severe allergic or inflammatory disease of the eye and adnexa, not the Pred Forte drop. Gastrointestinal lines are ulcerative colitis and regional enteritis for a critical stretch. Acute MS exacerbation is on the nervous-system line.

NHS takes it with breakfast

The NHS page on tablets and liquid says the usual adult daily amount sits between 5 mg and 60 mg, occasionally higher. Unless the prescriber writes another clock, take the day's prednisolone as one morning dose with breakfast so the stomach is less likely to complain and sleep is less likely to break. A 40 mg day can be eight of the 5 mg tablets at that one hour. Enteric-coated or gastro-resistant tablets swallow whole, with or without food; keep indigestion medicines two hours away from those coats. The UK 5 mg PIL from Accord tells the same swallow-with-water line and allows before or after a meal.

Courses under four weeks can stop

The Endocrine Society and European Society of Endocrinology joint guideline suggests no taper after glucocorticoid therapy shorter than 3 to 4 weeks, at any dose, because HPA suppression is a low concern and testing is not required for that stop. After a longer course the DailyMed tablet sheet wants a gradual reduction: drug-induced secondary adrenocortical insufficiency can last months after the last tablet, and a later stress (surgery, trauma, illness) may need the hormone put back. The lowest dose that still holds the disease is the labelled aim.

A 2024 taper primer in PMC slows once the patient is near a physiologic 5 mg prednisolone-equivalent a day: hold morning 5 mg, then 2.5 mg over two to four weeks, or switch to hydrocortisone 15 to 20 mg split, then step down. Morning serum cortisol, if used to confirm recovery, reads as a continuum; the joint guideline treats >10 μg/dL (300 nmol/L) as a safe-stop band and <5 μg/dL as a hold.

Appetite, mood, and infection

Orapred ODT's common corticosteroid reactions are fluid retention, a change in glucose tolerance, a rise in blood pressure, behavioral and mood changes, increased appetite, and weight gain. The mood list runs from euphoria, insomnia, and mood swings through personality change and severe depression to frank psychosis. Existing emotional illness can worsen.

The tablet sheet says corticosteroids, including prednisolone, suppress the immune system and raise infection risk from viral, bacterial, fungal, protozoan, or helminthic pathogens. They can hide some infection signs. Complication rates rise with the dose. Avoid the tablet in systemic fungal infection unless it is needed to control a drug reaction. People on immunosuppressant doses should stay away from chickenpox and measles; if exposed, get medical advice without delay. Do not vaccinate against smallpox while on corticosteroids. Other immunizations on high-dose steroid carry a neurological-complication warning and a weak antibody response.

A US pharmacy fills on a script

DailyMed prints Rx only on the 5 mg tablet, on Lannett's 15 mg/5 mL syrup, and on Pred Forte. Prednisolone acetate is not an OTC drop in the United States. Amazon Pharmacy will take an existing prescription by e-scribe to Amazon.com – Amazon Pharmacy Nationwide Home Delivery, or by transfer, then ship to a street address in all 50 states after a pharmacist review. Serv-U Pharmacy in Los Angeles states the same gate: a valid US prescription, generic for Prelone or Pediapred, pickup or local delivery. Compounded oral prednisolone from a.pharmacy verified shop such as CareFirst still needs a patient-specific script. Telehealth pages that advertise prednisone are a different INN; they do not fill this carton.

What Drugs.com printed for cash prednisolone

Drugs.com's prednisolone price guide, read 6 October 2026, listed 5 mg oral tablets from $72.50 for 30 tablets and from $231.75 for 100. The 15 mg/5 mL oral syrup started at $22.00 for 100 mL, $38.79 for 240 mL, and $67.59 for 480 mL. Sodium phosphate 5 mg/5 mL oral liquid started at $32.66 for 100 mL. Those are cash prints on that board, not a copay and not a promise at a named pharmacy counter.

Cats convert prednisone poorly

Veterinary Partner (VIN) writes that a dog's liver activates prednisone to prednisolone and the two are generally interchangeable in dogs. Cats convert less well, which is one reason prednisolone use has risen in feline practice. The UK VMD leaflet for a 5 mg veterinary tablet lists dogs and cats, starting 0.5 to 2.0 mg per kg per day for inflammatory and immune-mediated disease, then a step-down that can use alternate-day dosing. Thirst and urination are the classic salt-retention effects on that VIN page. After two weeks or more, VIN wants a taper toward every-other-day so the animal's own adrenal glands stay awake. A human 5 mg peach tablet is not a veterinary carton.
